But it's only partially actual lock downs. A large issue was hospitals and health care workers being too busy with Covid having to delay other procedures and appointments.

I understand, but it’s also disingenuous to say that medical staff “too busy” and ignore the fact that many people were straight up turned away or intentionally placed on waiting lists.

The major difference is a pandemic, lockdowns are only one of the consequences, the other, obviously, is that a lot of people got sick from covid, and when hospitals are overran with covid cases, they don't have time for routine operations like cancer screening.
