evulhotdog··on Nurses Sound Alarm as 'Uber for Nursing' Apps Push to Deregulate Healthcare
It’s pretty well known that vertical integration has been the issue with driving up costs. Salaries for physicians aren’t all that big of a cost, compared to all the administrative BS that hospitals and insurance companies drive up, and conveniently have agreements to heavily discount.

For example, on a roughly 400k surgery, maybe 4-5k of that goes to the providers (anesthesia and the surgeon), dependent one on the codes billed for the procedure.

There are private groups that purposely do not take insurance and bill purely out of pocket with a one time fee including all follow up’s and subsequent care, related to that procedure, and it is significantly lower (like 90%+ less) than what an average hospital is charging.

They aren’t public, so these are their own lines in the sand.

They are cash flow positive.

I guess capitalism just needs constant, unsustainable growth. Until your product and ideals are a hull of what it once was.

evulhotdog··on Herdr has released native app
I got access this week and I am blown away at how much better it makes oh-my-pi, and omp was already great!
evulhotdog··on Show HN: Specification First Agentic Development Plugin and Prove
I think this is where updating and maintaining the specs is just as important, if not more than adding new ones.

Maybe your flows coins take advantage of ensuring that the specs define the intent and active decisions made and not just “what the code does”, which it can actively read from the code itself.

I’ve recently adopted openspec for one of our repos and it’s worked great. Openspec is already decently well established and has a lot of support for all the different harnesses that exist.

I’m really starting to enjoy it for helping flush out the ideas in my head into something real and tangible.

evulhotdog··on A complete bypass of eMudhra’s 1FA OTP login system
I got the same, on a residential internet service. No idea what the trigger is, worked on my phone though.
evulhotdog··on Show HN: BetterWispr – Free, open-source dictation for Mac
I recently switched from Handy to Fluidvoice, and like it much better. The UI is much better and offers a better and more comprehensive feature set in comparison.

You don’t think the native mimic of Safari to open up the tab list is better?!

Don’t get me wrong, options are always a good thing!

From what I’ve read, a simple fan while sleeping checks the box for pink noise.
evulhotdog··on FreeTaxUSA's Binding Arbitration Agreement
Not that it makes it any better for new users, but for existing users, you can opt out of the arbitration by following the directions in the email they sent out.

evulhotdog··on Google Workspace thinks my domain is an email provider (2025)
I believe when the views of that person who has influence on the product, and it does not align with the services they say they desire to provide, it’s an issue.

It also can purely be choosing to not support an organization that doesn’t align with your world view. The money in our pockets is the power to change things.

evulhotdog··on The Corporate Creep of Plex: Why it may be time to move to Jellyfin
As I mentioned it’s pretty new and the server doesn’t even have a tagged or release version yet. Take a look at the repo activity: https://github.com/Silo-Server/silo-server/graphs/commit-act...

Marketing is not really the important bit at the moment when they’re working on building things from the ground up.

I was just mentioning it because it’s a new contender to the space, changing the status quo. Always a good thing in the OSS world.

evulhotdog··on The Corporate Creep of Plex: Why it may be time to move to Jellyfin
Silo is a new up and coming competitor, also completely open source and written in go, and has full Postgres support from the get-go, along with proper architecture decisions to allow transcoding across multiple hosts.

The vision of Silo appears to be much more clear, and has quite a few great devs behind it. It also has features to run side by side and has full JF api support. I have no involvement in the project, just excited for a new player in the space.

Something to keep an eye on!

evulhotdog··on OpenWrt One – Open Hardware Router
This is awesome, thank you for sharing.

evulhotdog··on Show HN: A memory database that forgets, consolidates, and detects contradiction
To me, the OP’s reply reeks LLM, along with many others from them in this thread.

I would hope that their replies are from an actual person, knowing they’re interacting with people in a similar field as themselves, and asking for criticism from real people in the top comment.

There’s plenty of options to get around that.
evulhotdog··on Attention lapses due to sleep deprivation due to flushing fluid from brain
Yeah they usually do have a dedicated sleeping space for their service. The thing is, they only sleep if there’s enough downtime. Depending on your service, size of the program, and of course who your patient population is, it could be a lot, or none at all.
evulhotdog··on Attention lapses due to sleep deprivation due to flushing fluid from brain
Current ACGME rules allow a max of 30 consecutive hours, so not as bad but still not great for someone you would hope to have fine motor skills to save a life!
evulhotdog··on Apple and F1 reach 5-year media deal for US broadcasts
Yep, 16.99 vs 12.99 USD.
evulhotdog··on Firefox profiles: Private, focused spaces for all the ways you browse
I tested this feature maybe 6 months ago. Have they copied the behavior of Chrome, where when you have multiple profiles open at once, it will open a link (say from Slack) in the most recently focused profile and window?

Last I checked, it didn’t, and it was quite annoying, coming from Chrome.

evulhotdog··on Purposeful animations
I’ve tried to find a way to reduce or eliminate those workspace switching animations and have not found a solution. I will say that using a hotkey to access it makes it go the fastest compared to swiping, but that’s about it. There are more extreme solutions like Flashspace but it’s not the greatest.
evulhotdog··on Reports of Gmail security issue are inaccurate
I think it’s a lot likelier that some other company which has both your phone and email was breached, and conveniently the domain in your email tells them who you use as your email provider, which they can then pose as.
